Thank you for your interest in Lifewise’s support for Individualised Funding! When we meet with you at the start, we will talk you through how to use Individualised Funding (IF).
We want to keep it as simple as possible for you, so depending on how you choose to use IF you may not need all of the information in this booklet.

What is INDIVIDUALISED FUNDING?
Individualised Funding (I.F.) is designed to enable you manage the specific support you need.
If you’re living with a disability and have I.F. from the Ministry of Health, Lifewise Homecare Services can work with you to establish the individually tailored support you require, including a choice of support worker as well as days and times suitable to your lifestyle.
You can have full control of all aspects of recruitment, employment and payment of your support worker or we can do some or all of this for you. We can provide support and coaching on all aspects of Individualised Funding.
We will maintain records of the support provided to you, monitor the quality of service delivery and complete the required reporting to the Ministry of Health.
You can also access our existing home based support services such as personal care, home support, buddying and community involvement as well as parenting courses. If you do not qualify for funded support, you can choose to access these services as a private client.
