The Lifewise Health and Disability Services can help you or your loved one lead an independent life in their own home.
Lifewise Support Workers help with a number of everyday tasks, even just being out and about. Our support is customised to suit your needs. This could be as little as a one-hour weekly visit or a daily plan of several hours.
The services that we provide are:
• Housework • Personal care • Shopping • Carer support • Respite
A Lifewise Support Worker’s assistance can include helping your loved one with a task, doing it for them, or teaching them how to do it for themselves. Our goal is to help them become as independent as they can be.
The issue (and the solution)
Growing older, with a disability or becoming unwell can be difficult to cope with. Faced with having to leave your own home as a result of these things is traumatic both for the individual as well as their family and loved ones.
Today, people faced with such difficulties lack the support traditionally provided by extended family or the local community. As a result, many become isolated and are unable to carry out the critical tasks required for independent living.
Homecare Services provides assistance with these critical tasks so more people can maintain their independence in the home or get back on their feet after illness or operations. Assistance can involve helping with a task, doing it for the person, or teaching them how to do it for themselves.
If you are living with a disability and have Individualised Funding, Lifewise will work with you to keep it simple and help you make the most of your support.
Individualised Funding gives people living with a disability choice about who supports them. It offers people more control over their support and gives people flexibility about how and when it happens.
Lifewise will also help you to know how to choose a suitable support worker and where you can go to find support workers.
You can have full control of paying your support workers or Lifewise can do this on your behalf for a small fee.
If you live within the Auckland DHB catchment area, you may also be able to access our home-based support services for personal care, household management and respite care.

You can have full control of all aspects of recruitment, employment and payment of support workers or Lifewise can do some or all of this for you. Lifewise can provide support and coaching on all aspects of Individualised Funding as well as providing information and links to community networks.
We will maintain records of the support provided to you, monitor the quality of service delivery and complete the required reporting to the Ministry of Health.
You can also access our existing home based support services such as personal care, home support, buddying and community involvement as well as parenting courses.
If you do not qualify for funded support, you can choose to access these services as a private client.
